A qualified coder must review, verify, and approve every code before it is used for documentation, billing, or claim submission. You remain fully responsible for the accuracy and compliance of the final codes (per the Australian Coding Standards, NPHIES, and CHI requirements).

Do not enter patient-identifying information — no names, National/Iqama ID, MRN, dates of birth, or contact details. Enter clinical text only. In line with the Saudi Personal Data Protection Law (PDPL), you are responsible for de-identifying notes before entry. Notes are processed only to generate codes and are not stored.
